The North-West Area Rehabilitation and Aged Care Service is based at two sites under the auspices of the Osborne Park Hospital Program.
The rehabilitation and aged care services located on the Osborne Park Hospital campus include:
- Inpatient clinical service (Rehabilitation and aged care - Wards 3, 4, 5)
- Inpatient consultation service
- Community Based Services
- Aged Care Assessment Team (ACAT)

- Inpatient Clinical Services (aged care - Ward C14)
- Surveillance Unit

Services Provided
The Rehabilitation and Aged Care Service located at Osborne Park Hospital provides comprehensive clinical, medical and rehabilitation services to the elderly population, and where appropriate, younger disabled residents within the catchment area of Osborne Park Hospital, by providing an optimal environment and physical facilities for:
- Comprehensive Assessment
- Rehabilitation and Restoration of function
- Treatment of Acute or Chronic Illnesses
- Provision of Domiciliary Services and Support
- Provision of Carer's Support
- Advice on Future Care
- Education
- Research.
Inpatient Clinical Services
The Rehabilitation and Aged Care Service at Osborne Park Hospital provides a comprehensive geriatric service to elderly patients residing within the catchment area in the areas of: medical treatment; management of Parkinson's Disease; and rehabilitation and consultative assessment including psychogeriatric, physiotherapy, occupational therapy, speech pathology, clinical psychology, and social and residential care assessment by social workers.
Patients can be admitted directly to the ward following referral by their general practitioner or other specialist, or as a transfer from another hospital such as Sir Charles Gairdner Hospital. The Service remains in contact with the patient's general practitioner for continuity of care.
Referrals
Referrals to the Rehabilitation and Aged Care Service are:
- Via General Practitioner
- Self referrals accepted, but we reserve the right to notify the client's General Practitioner and obtain appropriate relevant information
- Patients north of Beach Road may have their assessments at the Day Therapy Unit, Rehabilitation and Aged Care, Joondalup.
Aged Care Assessment Team (ACAT)
The ACAT is a Commonwealth Government initiative, which subsidises the operation of geriatric units in the assessment of the aged. This funding subsidises various positions within the Service. The ACAT program is fully integrated within the Rehabilitation and Aged Care Service and ensures a comprehensive and continuous service to the elderly. Click here for ACAT Catchment area.
Outpatient Services
These consist of:
- Ambulatory Diabetes Care Service
- Home visiting for aged care assessment
- Home aids/equipment assessment
- Residential care advice
- Acute medical/rehabilitation admission - including Occupational Therapy, Physiotherapy, Speech Therapy, Dietetics
- Day Hospital assessments (Email)
- Care Awaiting Placement
- Memory evaluation
- Osteoporosis assessment/Falls evaluation
- Continence advice
- Parkinson's disease evaluation.
Day Hospital
The Day Hospital provides a number of outpatient clinics and runs three groups for specific clients:
- Stroke Group
- Parkinson's Group
- Lee Silverman Voice Technique (through Speech Pathology)
Medical appointments where patients are reviewed by the consultant geriatricians and junior medical staff are conducted daily.
A Parkinson's Disease Clinic is conducted three times a week for newly diagnosed and those patients with long-standing Parkinson's disease. Medical and Allied Health staff review the patient to determine an appropriate treatment and management program. The Parkinson's Disease Clinic staff include the medical consultants and junior medical staff, a social worker, occupational therapist, physiotherapist, and speech therapist.
A Falls Clinic to assess a patient's potential falls risk is conducted once a week and provides a detailed assessment and recommendations for older patients who are at risk of falls.
The Memory Evaluation Clinic provides a detailed assessment and treatment for people who may be experiencing difficulties with their memory. The clinical psychologist and medical consultants provide a specialised service for patients referred to the Memory Evaluation Clinic. |
