Outpatient consultations are only available to patients who live within the NMAHS region with a written referral from their GP. An appointment letter will normally be sent to your patient within 10 working days of the referral being received. Outpatient clinics are provided at Osborne Park Hospital.

Referrals for hypertension, impaired glucose tolerance, overweight and high cholesterol are invited to attend the Healthy Lifestyle Program. Exceptions to these are for children and those with mental health issues which prevent group participation.

1 to 1 appointments are generally reserved for those with:

  • Gastrointestinal problems (e.g. coeliac disease, ulcerative colitis, diverticular disease)
  • Morbid obesity
  • Allergies
  • Children's nutritional problems
  • Type 2 diabetes requiring insulin
  • Cancer
  • Parkinson's disease

We welcome referrals from you and have 3 versions of referral templates available for your use:

Alternatively if you prefer to send your own electronic form please include the following information:

  • Reason for Referral
  • Diagnosis
  • Relevant past medical history
  • Medications
  • Allergies
  • Relevant investigations (eg. cholesterol, BSL, FBP, Fe)

Healthy Lifestyle Program

Our Healthy Lifestyle Program is run by our dietitians and physiotherapists to help make permanent healthy lifestyle changes. It is for both men and women providing advice on:

  • Healthy weight loss
  • High cholesterol
  • High blood pressure
  • Blood sugar control and
  • Exercise

It is free to anyone living in the North Metropolitan area.

Please fax your referral to 9346 8232 or ask patients to self refer by calling 9346 8128.

In Medical Director, open Letter Writer, open Blank Template, select Import to import this document, select Save As to save this template, use the template by selecting it when you create a New Letter.

Instructions for importing into other software

We welcome use of this form or your own computer generated version. In view of the number of different software products available we are unable to provide specific templates for each product. However you may use the Word version to import the document into your own medical software. You will need to create the automated fields such as patient and doctor details.
