OPH performs the following GI endoscopy procedures as day surgery using the Ambulatory Surgery Initiative (ASI) for patients with a valid Medicare card:

  • gastroscopy,
  • sigmoidoscopy and
  • colonoscopy

In order for to provide your patient with the opportunity to be treated under the ASI Ambulatory Surgery Initiative referral optionat OPH, a referral to a named specialists is required. You may select more than one of our specialists but you must name each of the chosen specialists for your referral to be valid under ASI. To facilitate referral we have 3 versions of referral templates available. We also welcome electronic referrals which include the required information using your own template.

Alternatively the generic ASI referral form is available at http://www.gp.health.wa.gov.au/asi/forms/referral.aspx. You will need to select Osborne Park Hospital and Gastroenterology. Select the gastroenterologist of your choice and a form with the specialist's details will be automatically generated for you. However this form does not interact with medical software programs and does not import your patient details or yours and you will need to enter this information manually.

There are no geographic exclusion criteria. However OPH provides low risk surgical procedures only. Please note the following exclusion criteria for surgical procedures at OPH:

  • weight greater than 120 kg / BMI > 40 or other high anaesthetic risk factors

In addition to the Ambulatory Surgery Initiative a number of specialists also have access to OPH for GI endoscopy procedures. To refer to these specialists please refer to their rooms. These are listed in Specialists with Admitting Rights to OPH.

Instructions for importing into the Medical Director software

In Medical Director, open Letter Writer, open Blank Template, select Import to import this document, select Save As to save this template, use the template by selecting it when you create a New Letter.

Instructions for importing into other software

We welcome use of this form or your own computer generated version. In view of the number of different software products available we are unable to provide specific templates for each product. However you may use the Word version to import the document into your own medical software. You will need to create the automated fields such as patient and doctor details.
