Community Aids and Equipment Program (CAEP)

About CAEP
CAEP is a Disability Services Commission funded program to provide home modifications and loan of aids and equipment for people with long term disabilities with the goal of enabling people to live in their own homes within the community. Usually the service is provided in your patient's home, however this will depend on the equipment required and the health professional involved.

Inclusion criteria
  • disability of a permanent or indefinite nature (including physical, neurological, sensory, cognitive or intellectual, psychiatric); and
  • has one of the following:
    • pensioner concession card, or HCC; or
    • Commonwealth (not state) Seniors Health Care card; or
    • Carers payment or are eligible for this; or
    • demonstrable financial hardship
  • lives at home in the community or in a group home, or state funded Nursing Home such as Brightwater Care , CPA or Rocky Bay.
  • has not received a compensation settlement; and
  • not eligible for equipment through other funding programs, e.g. DVA.
Exclusion criteria
  • hospital inpatient, outpatient or day patient - within the last 3 months.
  • in receipt of any Commonwealth Aged Care funding, including the following:
    • being a resident in a high or low care facility (nursing home/hostel) or
    • receiving a Commonwealth Aged Care Package – eg Community Aged Care Package (CACP) or Extended Aged Care at Home (EACH).

A referral is required by the General Practitioner prior to assessment.  For some equipment a specialist referral will also be required.

Types of Equipment provided
CAEP will only provide basic and essential equipment that meets a person's essential functional need. CAEP does not fund equipment for therapeutic benefit. Examples of equipment provided include:

  • Manual and electric wheelchairs
  • Hoists
  • Shower and toilet equipment and rails
  • Home modifications
  • Footwear and orthotics
  • Walking frames and braces
  • Communication aids

Types of service provided

  • Assessment of equipment requirements
  • Provision of equipment and/or organisation of home modification
  • Training of client and caregivers in use of equipment
  • Follow-up assessment of equipment
  • CAEP does not provide therapeutic intervention

Aged Care Assessment Team (ACAT)

ACAT  Occupational Therapists  provide a home assessment service and provide intervention (including equipment provision) and screening for all OPH Rehabilitation and Aged Care (RAC) services, to clients aged over 65 years, who have been referred to the service by their GP.

Parkinson's Disease Clinic (PDC)

The PDC Occupational Therapists assess the impact of Parkinson's disease symptoms on your patient's abilities and provides intervention to improve function and safety. Patients aged over 65 years with Parkinson's disease who have been referred by their GP are eligible.

Day Hospital

An Occupational Therapy rehabilitation service is provided to Day Hospital attendees 65 years and over who have been referred by their GP. Home based intervention is also provided.

RAILS (Rehabilitation & Aged Care Intervention & Liaison Service)

RAILS is a multidisciplinary rapid response team based at OPH – Department Rehabilitation and Aged Care . RAILS focusses on reducing in-patient length of stay at OPH, preventing avoidable Emergency Department admissions and preventing re-admission of discharged patients. RAILS is able to provide a same day service for urgent referrals.
