The following rehabilitation and aged care services are available to assist GPs with the care of older patients:

24 Hour Access Geriatrician Telephone Support

Medical practitioners may contact our on-call geriatrician via the OPH switchboard on 9346 8000.
Our on-call geriatricians can provide clinical advice, arrange day hospital reviews and direct admissions when appropriate.

Urgent home visiting for those at risk of hospital admission is provided by the Rehabilitation and Aged Care Intervention Liaison Service (RAILS) which provides the following services to reduce the risk of ED referrals:
- review and comprehensive care planning for patients at home,
- facilitates direct admissions and day hospital reviews and
- increase support for patients after discharge.
An urgent review can be arranged for the same day. To refer please call the RAILS nurse on 9346 8315 or 0404 803 569, who can facilitate access to the multidisciplinary team including occupational therapist, physiotherapist, social worker and geriatrician. You may also fax your referral directly to RAILS on (08) 9346 8263.
This number is intended to assist health professionals in referring and accessing information. Please do not give it out.

Routine referrals to the Rehabilitation and Aged Care Services include referrals to the following:
- Aged Care Assessment Team
- (for assessment for residential care and community care services)
- Update of ACAT approval (now known as ACCR)
- Medical assessment
- Memory clinic
- Falls clinic
- Parkinson's clinic (includes clinical nurse, social worker, physiotherapist, occupational therapist, therapy assistant, speech therapist and access to clinical psychologist)
- Day Hospital Outpatient Clinics for general medical assessment
- Social work
- In-home support services
- Day centre/social support for social isolation issues
- Respite care
- Guardianship issues
- Carer stress issues
- Occupational therapy services
- Physiotherapy services
- Nursing services
- Continence clinic
- Speech Therapy services
- Dietitian services
- Diabetes service
We welcome referrals from you and have 3 versions of referral templates available for your use:
- (Medical Director Template 18 Kb) Instructions for importing into Medical Director software.
- (DOC 50 Kb) Instructions for importing into other medical software.
- (PDF 50 Kb) For handwritten referrals.
Alternatively, a computer generated list from your software program is accepted and welcomed.
Please fax your referral to (08) 9346 8232. Information required at referral includes the following:
- Patient demographic information including contact details of significant other if possible;
- Medical history, medications, drug reactions, if memory issues please include most recent MMSE if done;
- Indication of services required.
Instructions for importing into the Medical Director software
In Medical Director, open Letter Writer, open Blank Template, select Import to import this document, select Save As to save this template, use the template by selecting it when you create a New Letter.
Instructions for importing into other software
We welcome use of this form or your own computer generated version. In view of the number of different software products available we are unable to provide specific templates for each product. However you may use the Word version to import the document into your own medical software. You will need to create the automated fields such as patient and doctor details.
