Urology Day Surgery

Working out of OPH, Dr Christopher Rowling provides the following procedures as day surgery for patients with a valid Medicare card:

  • cystoscopy, and
  • other minor urology

We welcome referrals from you and have 3 versions of referral templates available for your use:

You may also send your own electronically generated form if you prefer. Please identify clearly the specialist and procedure selected, as well as the patient's demographic details (name, date of birth, address, telephone) and relevant clinical information (medical history, medication and drug reactions).

Alternatively the generic ASI referral form is available at http://www.gp.health.wa.gov.au/asi/forms/referral.aspx. You will need to select Osborne Park Hospital and Urology. The specialist's details will be automatically generated for you in this form. However this form does not interact with medical software programs and does not import your patient details or yours and you will need to enter this information manually.

Please fax the referral to the number for the individual specialist as indicated on the ASI website above. If you use the automatically generated form, the fax number will be on the referral.

There is no geographic exclusion criteria. However OPH provides low risk surgical procedures only. Please note the following exclusion criteria for surgical procedures at OPH:

  • weight greater than 120 kg or other high anaesthetic risk factors

Instructions for importing into the Medical Director software

In Medical Director, open Letter Writer, open Blank Template, select Import to import this document, select Save As to save this template, use the template by selecting it when you create a New Letter.

Instructions for importing into other software

We welcome use of this form or your own computer generated version. In view of the number of different software products available we are unable to provide specific templates for each product. However you may use the Word version to import the document into your own medical software. You will need to create the automated fields such as patient and doctor details.
