Social Work
Social workers are trained professionals who work within a multi-disciplinary team which comprises doctors, nurses and allied health professionals.
Social workers are available to help clients and their families manage the practical, personal and emotional issues which arise due to acute or chronic illness. Their primary roles are to provide counselling, link clients into community resources to help them manage at home and provide information on community agencies and resources which can assist them.
Service Structure
Social workers are trained professionals. The Coordinator of Social Work Services is responsible for the day to day management of social work services at Osborne Park Hospital. Services are provided weekdays only. The service is free.
Inpatient Services
Services are provided to all in patients of the hospital. Referrals may be accepted directly from patients and their families or through their treating doctor.
Outpatient Services
Services are provided to clients who are already attending the Antenatal and Parkinson's Clinics and the Rehabilitation & Aged Care Day Hospitals.
The Rehabilitation & Aged Care service also provide an Aged Care Assessment Team (ACAT) role, whereby general practitioners refer community clients to the service. Social workers are attached to this team.
