Patient Information Services

A comprehensive medical record is maintained for every patient attending OPH. The record is a summary of clinical events during the person's stay.

The medical record is the most important communication tool between clinicians involved in treating a patient. Patient Information Services staff at OPH are committed to assisting clinicians achieve a high standard of documentation, while safeguarding patient confidentiality and privacy.

Patients may apply for access to their medical record under the Freedom of Information Act.

Freedom of Information

The Freedom of Information Act 1992 provides the public of WA with the right to apply for access to information held by government agencies. This includes medical records.

The usual form of access is the release of a photocopy of the documents. Medical records can only be accessed with patient consent.

Applications are made in writing to the Coordinator, Patient Information Services at Osborne Park Hospital.
