The North-West Area Rehabilitation and Aged Care Service is based at two sites under the auspices of the Osborne Park Hospital Program.
The rehabilitation and aged care services located on the Osborne Park Hospital campus include:
- Inpatient clinical service (Rehabilitation and aged care - Wards 3, 4, 5)
- Inpatient consultation service
- Community Based Services
- Aged Care Assessment Team (ACAT)
- Outpatient Services
- Day Hospital.

Services Provided
The Rehabilitation and Aged Care Service located at Osborne Park Hospital provides comprehensive clinical, medical and rehabilitation services to the elderly population, and where appropriate, younger disabled residents within the catchment area of Osborne Park Hospital, by providing an optimal environment and physical facilities for:
- Comprehensive Assessment
- Rehabilitation and Restoration of function
- Treatment of Acute or Chronic Illnesses
- Provision of Domiciliary Services and Support
- Provision of Carer's Support
- Advice on Future Care
- Education
- Research.
Philosophy
We believe that the best care for people with multiple problems of a medical, social, functional and psychological nature is provided by a highly skilled professional team with varying degrees and areas of expertise. We recognise the rights of individuals to make decisions, which affect their future and autonomy, and to maintain their identity, dignity and privacy. We respect the wishes and social, cultural and religious values of patients and their families and believe that patients have the right to be informed about the care process, to decide whether or not to participate in it, and if in doubt to seek the opinion of others.
Inpatient Clinical Services
The Rehabilitation and Aged Care Service at Osborne Park Hospital provides a comprehensive geriatric service to elderly patients residing within the catchment area in the areas of: medical treatment; management of Parkinson's Disease; and rehabilitation and consultative assessment including psychogeriatric, physiotherapy, occupational therapy, speech pathology, clinical psychology, and social and residential care assessment by social workers.
Patients can be admitted directly to the ward following referral by their general practitioner or other specialist, or as a transfer from another hospital such as Sir Charles Gairdner Hospital. The Service remains in contact with the patient's general practitioner for continuity of care.
Catchment Area
North of Karrinyup Road - Morley Drive and west of Alexander Drive including Ballajura - postcodes 6018, 6020, 6021, 6022, 6023, 6024, 6025, 6026, 6027, 6028, 6029, 6030, 6031, 6032, 6033, 6035, 6037, 6061, 6064, 6065, and 6066.
Clients not located in any of the above postcodes can contact our Service on 9346 8111 for advice on their appropriate geriatric service.
Referrals
Referrals to the Rehabilitation and Aged Care Service are:
- Via General Practitioner
- Self referrals accepted, but we reserve the right to notify the client's General Practitioner and obtain appropriate relevant information
- Patients north of Beach Road may have their assessments at the Day Therapy Unit, Rehabilitation and Aged Care, Joondalup.
Aged Care Assessment Team (ACAT)
The ACAT is a Commonwealth Government initiative, which subsidises the operation of geriatric units in the assessment of the aged. This funding subsidises various positions within the Service. The ACAT program is fully integrated within the Rehabilitation and Aged Care Service and ensures a comprehensive and continuous service to the elderly.
Outpatient Services
These consist of:
- Home visiting for aged care assessment
- Home aids/equipment assessment
- Residential care advice
- Acute medical/rehabilitation admission - including Occupational Therapy, Physiotherapy, Speech Therapy, Dietetics
- Day Hospital assessments (Email)
- Care Awaiting Placement
- Memory evaluation
- Osteoporosis assessment/Falls evaluation
- Continence advice
- Parkinson's disease evaluation.
Day Hospital
The Day Hospital provides a number of outpatient clinics and runs three groups for specific clients:
- Stroke Group
- Parkinson's Group
- Lee Silverman Voice Technique (through Speech Pathology)
Medical appointments where patients are reviewed by the consultant geriatricians and junior medical staff are conducted daily.
A Parkinson's Disease Clinic is conducted three times a week for newly diagnosed and those patients with long-standing Parkinson's disease. Medical and Allied Health staff review the patient to determine an appropriate treatment and management program. The Parkinson's Disease Clinic staff include the medical consultants and junior medical staff, a social worker, occupational therapist, physiotherapist, and speech therapist.
A Falls Clinic to assess a patient's potential falls risk is conducted once a week and provides a detailed assessment and recommendations for older patients who are at risk of falls.
The Memory Evaluation Clinic provides a detailed assessment and treatment for people who may be experiencing difficulties with their memory. The clinical psychologist and medical consultants provide a specialised service for patients referred to the Memory Evaluation Clinic.
Medical Staff
The physicians provide:
- a consultant service for acute inpatient admissions within teaching and non-teaching hospitals including assessment and management
- a consultative service to other physicians and surgeons within the area hospitals, and
- a domiciliary physician service to general practitioners.
Clients remain under the care of their general practitioner for their general health. Medical reviews are done on request from the general practitioner or from the consultant physician.
Dr Barry Vieira, MB BS FRACP is the Clinical Head of Service for Osborne Park Hospitalis Departement of Rehabilitation and Aged Care. He is an accredited consultant physician for Joondalup Health Campus, Sir Charles Gairdner Hospital, Glengarry Hospital and Osborne Park Hospital.
Dr Vieira obtained his Fellowship to the Royal Australian College of Physicians in Geriatric and General Medicine in 1987 before commencing at Osborne Park Hospital in 1990.
His main areas of interest include:
- Management of long-standing Parkinsonis Disease
- Cardiovascular medicine in the elderly, particularly hypertension and heart failure
- Administration and policy planning for services impacting on the elderly within the North Metropolitan Area Health Service catchment area
- Palliative Care Medicine.
Dr Nicole Lothe, MB ChB FRACP, is a consultant physician in Rehabilitation and Aged Care. She is an accredited consultant physician at Sir Charles Gairdner Hospital, Glengarry Hospital, Joondalup Hospital and Osborne Park Hospital and has a special interest in osteoporosis and Alzheimer's disease. She also works with the Parkinsonis Disease Clinic through the Osborne Park Hospital Program.
Dr Larry Liew, MB BS FRACP, is a Consultant Physician in Rehabilitation and Aged Care. He is an accredited consultant physician for Joondalup health campus, Glengarry hospital and Osborne Park Hospital. He has a special interest in palliative care and pain management.
Dr Jake Harper, MB ChB FRACP is a Consultant Physician in the Rehabilitation and Aged Care. He is an accredited consultant physician for Joondalup Health Campus and Osborne Park Hospital. He works in the Rehabilitation and Aged Care area with a special interest in Parkinsonis disease. He is one of four consultants who work within the Parkinsonis disease clinic at Osborne Park Hospital.
Dr Andrew Granger, MB ChB FRACP is a Consultant Physician in the Rehabilitation and Aged Care. He is an accredited consultant physician for Joondalup Health Campus, Sir Charles Gairdner Hospital and Osborne Park Hospital. He works in the Rehabilitation and Aged Care area with a special interest in Parkinsonis disease. He is also one of four consultants who work within the Parkinsonis disease clinic at Osborne Park Hospital.
Nursing Staff
The nursing staff provide individualised nursing care for all the patients in a safe and caring environment, and at a standard of excellence by:
- understanding and supporting the patients and their families during illness and recovery and rehabilitation;
- providing coordinated care by establishing and maintaining effective communication between all team members and relevant family members/carers.
The Home Visiting nurse is responsible for the supply and arrangements for installation of standard home aids to enable patients to continue independent living at home e.g. bathboard, shower drain, shower hose, bedside rails, commode, toilet frame, chair.
The Home Visiting nurse visits clients in their home environment, and carries out a full assessment.
Allied Health Staff
Assessment, rehabilitation, treatment and support services are provided by allied health staff from a range of disciplines, including: clinical psychology; dietetics; occupational therapy; physiotherapy; podiatry; social work, and speech pathology.
