RAILS – Rapid Response Team

The RAILS multidisciplinary team provides rapid response service for people over the age of 65 years, living in the North Metropolitan Health Service's catchment area (PDF). RAILS conducts comprehensive care planning and home visits.

The purpose of this service is to:

• Reduce the inpatient length of stay for OPH geriatric, acute and rehabilitation medicine inpatients.
• Provide urgent intervention to community geriatric referrals to prevent hospital admissions or readmissions.
• Follow-up at risk patients discharged from OPH GARM wards, to avoid re-admissions.

The team comprises a consultant geriatrician, clinical nurse, senior social worker, senior occupational therapist and senior physiotherapist.

RAILS Referrals

A written doctor’s referral is required and can be faxed direct to the RAILS multidisciplinary team at Osborne Park Hospital.
